		<description>Several HR Professionals and Attorney’s who specialize in Labor &amp; Employment Law have stated that the on-line forms placed by NY State are misleading, ask questions that don’t pertain to the WTPA and could, if answered incorrectly open the employer up to additional fines and/or penalties. That being the case we strongly urge you to seek qualified assistance in complying with this act.

In addition, NY State has another new law, simply make sure your companies telephone number appears on the Pay Check and on the Pay-stub. Again penalties are there to make NYS money.</description>

Worse yet, they don&#039;t seem to be bothered that this new law and the way that the law will be used, THIS COULD COST YOUR COMPANY MONEY, as the State is going to use this Act for revenue generation.

Another nice factoid is you can build your own form, as long as it contains the same data as the sample forms; tying it into a database.  Payroll companies may or may not be providing these forms, but we still suggest you spread the word.  

Note:  All new hires since April 9th, 2011 require this form, as well as when a raise occurs. Also, ALL employees need to sign the form in January 2012 and then every anniversary of January.

The State is being sneaky - by not really informing companies in the State about the requirements and the stiff penalties (up to $2500 per employee, per year).
